Adult Day Health

To provide a variety of health, therapeutic and social activities designed to serve specialized needs of those at risk of placement in a nursing facility. Restore or maintain optimal capacity for self care to frail elders and other physical impaired adults. Prevent or delay placement into long term care facility.  To create a partnership with the participant, family, physician and the community in working toward maintaining personal independence.

Making the decision to seek out help for yourself or a loved one can be difficult but participants and families soon discover that the program is ripe with benefits, including:


  • Nutritional meals and snacks that may be hard to prepare on your own at home.

  • Socialization, reducing feelings of isolation and loneliness.

  • Comfort in meeting others with similar health or physical limitations.

  • Close connections to the health care services, including CCSP, SOURCE and other services.

  • And most importantly peace of mind that you or your loved one are safe.


We are licensed by the Georgia Department of Community Health and accept Medicaid.
