Adult Day Health

To provide a variety of health, therapeutic and social activities designed to serve specialized needs of those at risk of placement in a nursing facility. Restore or maintain optimal capacity for self care to frail elders and other physical impaired adults. Prevent or delay placement into long term care facility.  To create a partnership with the participant, family, physician and the community in working toward maintaining personal independence.
