We are committed to providing high quality, multidisciplinary care by professionals who recognize the need for comprehensive assessment of needs from both the client and professional's point of view.
To provide a variety of health, therapeutic and social activities designed to serve specialized needs of those at risk of placement in a nursing facility. Restore or maintain optimal capacity for self care to frail elders and other physical impaired adults. Prevent or delay placement into long term care facility. To create a partnership with the participant, family, physician and the community in working toward maintaining personal independence.
Structured Family Caregiving service provides support to family caregivers who live with and assist an elderly and/or disabled waiver participant requiring significant assistance with activities of daily living.
Eligibility for this service option is contingent on there being a live-in, unpaid family caregiver in the home and an assessed need for a minimum of five hours of personal support service daily.
